Background

Community Health (and Care) Partnerships represent a major new configuration for primary care in Scotland and they are expected to improve health and reduce health inequalities in local communities. A population focus, a wider perspective on health and disadvantage, and a requirement to address health inequalities are all new arenas for mainstream primary care structures. CHPs have untested potential for reducing health inequalities within and across patient populations.  

This programme aims to facilitate the use of evidence-based knowledge and innovative thinking to the development of the Glasgow CHP/CHCP strategy for reducing and preventing health inequalities. It will work wherever possible with other programmes and structures in the Centre for Population Health, NHS Greater Glasgow and Clyde, the University of Glasgow and Glasgow Community Planning in order to maximise use of resources and potential for influencing change. Initially, the programme carried out exploratory studies of different elements of CHP/CHCP roles in addressing health inequalities, and between 2007 and 2009 will work on developing a framework for CHP/CHCPs to monitor and review health inequalities action.

 

Aim

To maximise the impact of Community Health Partnerships on health improvement and health inequalities by generating, reviewing and supporting the implementation of evidence, and building capacity for understanding and action.

Milestones

Projects incorporated in the programme include:
  • Project 1 : Managing partnerships for population health improvement
  • Project 2 : Inequalities, gender sensitivity and primary care mental health
  • Project 3 : Children, inequalities and information
  • Project 4 : Smoking during pregnancy
  • Project 5 : GPs and social prescribing pilot
  • Project 6 : Primary care response to inequalities and mental health
  • Project 7 : Developing a CHP monitoring framework for health inequalities
  • Project 8 : Policy background for CHPs and CHCPs

Projects have independent timescales and reports. Information is posted on this website at the links above as it becomes available.

Also associated with this programme of work is the development of new community profiles.
